The position supports PATH’s Family Services team at the Family Solution Center, helping people experiencing homelessness find housing options and services. The coordinator integrates problem-solving interventions into family services, helping guests address housing crises and identify alternatives outside the homeless crisis response system. The role combines case management, staff training, referrals, assessments, and coordination with partner systems to support guests’ housing and service goals.
What you'll do
- Support case managers in interim housing with problem-solving integration
- train Family Solution Center staff
- coordinate with the LAHSA Problem-Solving Unit
- maintain problem-solving tracking and partner-system integration
- process referrals
- meet assigned guests at least weekly
- conduct intakes and needs assessments with supervisor or MSW/LCSW staff
- help develop individualized service plans
- facilitate psychosocial rehabilitation groups as required
- complete VI-SPDAT assessments and HMIS entries
- support benefits, identification, employment, housing, and health-related needs.
