• Conduct initial and ongoing psychosocial assessments
• identify emotional, social, financial, and environmental needs
• provide counseling, crisis intervention, and short-term supportive therapy
• educate patients and caregivers about coping strategies and community resources
• develop patient-centered care plans
• coordinate community resources
• collaborate with the interdisciplinary team
• provide patient visits, assessments, follow-ups, and discharge-planning support as requested.
