What you'll do

• Set attainable client goals and monitor progress
• identify barriers and ways to address them
• develop housing plans
• assist with housing searches, veteran assistance, information, and referrals
• follow up on client progress and report to management
• document goals and services
• enter required information into the Pinellas Homeless Information Network
• connect clients with services and community resources
• support teamwork and collaboration.
