The role includes individualized treatment planning, individual, group, or family therapy, crisis intervention when needed, clinical documentation, and coordination with residents’ care teams, families, and facility staff.

What you'll do

• Conduct psychosocial assessments
• develop, implement, and monitor treatment plans
• provide individual, group, and/or family therapy
• address depression, anxiety, grief, cognitive decline, and adjustment to illness or disability
• support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns
• collaborate with clinical and facility teams
• participate in care plan meetings
• maintain compliant clinical documentation
• provide crisis intervention and behavioral management strategies when needed
• educate staff and families.
