What you'll do

• Consult with patients and families about hospice services and coordinate admissions. Assess patients at admission and arrange initial nursing-visit frequency and durable medical equipment and prescription needs
• assess needs for bereavement, social work, chaplain, and volunteer services. Report to the interdisciplinary team and complete admission paperwork and charting. Nursing or tuck-in visits, training admission staff, and supervisory visits may be performed.
