• Perform in-home assessments and skilled nursing visits

• provide wound care, disease management, medication administration, and patient education
• communicate with physicians, case managers, and interdisciplinary teams
• complete timely regulatory documentation
• participate in an on-call rotation as needed
• and build relationships with patients and families.
