Lead Case Manager – Residential Programs

The Lead Case Manager – Residential Programs role is onsite in Paterson, NJ, with a Monday–Friday schedule from 8:30 a.m. to 4:30 p.m. The stated pay is $20–$23 per hour. The role supports Straight and Narrow, a program providing residential and outpatient services to people experiencing substance use and co-occurring disorders. The case manager provides social-service case management to residential treatment clients and helps coordinate services, benefits, referrals, and discharge planning. The lead also trains case managers, coordinates with referral and social-service agencies, maintains client records, and supports treatment-team goals.

What you'll do

• Manage a caseload of residential and outpatient addiction-treatment clients
• train new case managers and residential-life staff
• coordinate with courts, probation, child-protection, and social-service agencies
• help clients obtain identification and access benefits
• arrange appointments and referrals
• coordinate case conferences
• support discharge planning
• facilitate scheduled groups and didactics
• document client sessions and maintain charts
• attend case-management and multidisciplinary meetings
• provide crisis intervention as needed
• follow electronic health record policies and professional boundaries.

Why this role
