This role supports the Substance Use Disorder Care Management program, which provides short-term care management and treatment linkage for adults and adolescents. The purpose of this role is to conduct assessments, develop client-centered plans of care, and coordinate services for individuals with complex psychiatric, substance use, or co-morbid medical conditions. The Care Manager ensures access to physical and behavioral health services while facilitating communication between providers to improve client outcomes.

What you'll do

• Conducts assessments and develops clinical plans of care
• performs care management for an assigned caseload
• tracks and monitors client progress
• develops resource inventories
• provides linkage and referral to service providers
• facilitates case conferences
• provides advocacy and education to clients and families
• maintains accurate case records in the EMR.
