What you'll do

• Verify the plan of care with the RN before each visit
• collaborate with physicians and RNs and report changes in patient condition
• teach patients and caregivers self-care techniques
• check vital signs
• review medications and complete medication reconciliation at each visit
• perform physician-ordered procedures such as Foley catheter changes
• prepare treatment equipment using aseptic technique
• and complete documentation accurately and on time. Requires a current state-specific LPN license, CPR certification, at least one year of LPN/LVN experience, and a valid driver's license, reliable transportation, and liability insurance.
