Social Worker

The Social Worker role is onsite in Crossville, TN, with compensation of $24–$26 per hour. WyndRidge Health and Rehabilitation Center provides resident care in a rehabilitation setting. This role promotes residents’ psychosocial well-being through emotional support, individualized care planning, advocacy, and coordination of safe discharges. The Social Worker works with residents, families, physicians, and the interdisciplinary team to support person-centered care and regulatory compliance.

What you'll do

• Conduct psychosocial assessments
• develop and update individualized care plans
• provide emotional support, counseling, and crisis intervention
• connect residents and families with community resources
• coordinate discharge plans and post-discharge services
• advocate for resident rights
• participate in care conferences
• document services and support regulatory compliance.
