What you'll do

• Provide complex case management

• review case information, conduct assessments, and provide diagnoses under LCSW-C supervision and treatment recommendations

• identify and address reentry barriers
• develop and monitor treatment plans with the multi-vendor team
• coordinate referrals and benefits
• provide psychoeducation and counseling
• conduct individual and group counseling focused on substance use treatment
• participate in meetings, training, research, and quality improvement.
