What you'll do
- Complete and submit accurate MDS assessments
- coordinate care plan conferences
- keep assessment reference dates within Medicare and Medicaid guidelines
- participate in quality measure meetings
- work with nursing leadership to address documentation opportunities
- and assist the interdisciplinary team with Medicare coverage decisions.
