What you'll do

• Provide skilled home care and comprehensive assessments, including OASIS documentation
• develop and adjust individualized care plans
• monitor patient condition and report changes to physicians
• educate patients and caregivers on medications, treatments, disease management, and home safety
• coordinate with therapists, aides, and other providers
• prepare for discharge and document care in the EMR.
