What you'll do

• Conduct psychosocial assessments
• develop, implement, and monitor individualized treatment plans
• provide individual, group, and/or family therapy
• address depression, anxiety, grief, cognitive decline, and adjustment to illness or disability
• collaborate with clinicians, physicians, facility staff, social workers, and rehabilitation teams
• participate in care planning
• maintain compliant clinical documentation
• provide crisis intervention and behavioral management strategies when needed
• and educate staff and families.
