Job overview

The Health Partner role is based at Daymark Health’s Providence, RI office five days per week, with occasional in-home patient visits; annual compensation is $50,000–$60,000. The role helps patients and caregivers navigate care while coordinating with Daymark and external providers.

What you'll do

• Conduct structured social-needs assessments and connect patients with community resources
• support care plans and case conferences
• prepare patients for virtual visits and troubleshoot basic technology
• schedule follow-up visits and referrals
• prepare charts
• track symptom surveys and follow up
• escalate clinical concerns
• document interventions and coordination in the electronic health record.

Why this role

Requires at least three years of experience as a medical assistant, CNA, EMT, or in a similar clinical or care-coordination setting. Oncology experience is a bonus; in-home visits require a valid driver’s license.
