Healthcare Consultant I — Care Coordinator

What you'll do

• Evaluate referred members’ needs and eligibility using care management tools and available program information
• identify risks and service needs
• coordinate and monitor care plan activities
• consult with case managers and other health and behavioral health programs
• collaborate with Child and Family Teams
• escalate quality-of-care issues
• help secure appropriate services
• support member engagement and informed healthcare decisions
• document care in accordance with regulatory and company requirements.

Why this role
