What you'll do

• Coordinate case management and supervise services staff
• provide orientation, assessments, education, supervision, and evaluation to assigned staff and volunteers
• monitor required supervisory visits
• develop and update care plans and documentation
• provide skilled nursing care and initial assessments
• participate in the interdisciplinary team
• serve on a rotating subject-to-call basis as assigned.
