This role supports the home health department at AccentCare, Inc., a provider dedicated to delivering compassionate care to patients in their own homes. The role exists to provide specialized psychiatric and mental health nursing care to patients managing physical and mental illnesses, late-life depression, and substance abuse concerns. The nurse is responsible for conducting patient assessments, establishing and managing care plans, coordinating with physicians, and providing clinical supervision to LVNs and home health aides to ensure high-quality patient outcomes.
What you'll do
- Assessment of nursing needs, providing direct care and case management of the patient's care plan.
- Makes initial assessment and reassessment visits utilizing observation/assessment skills.
- Informs the physician of the results of the assessment and recommended clinical interventions.
- Documents and submits the assessment, progress and discharge notes and other required paperwork.
- Manages medical illness and performs psycho-biological interventions.
- Evaluates medicines to identify possible ineffective drug therapy, adverse reactions, and contraindications.
- Instructs regarding ordered medication regimen and reviews medications and compliance.
- Administers IM medications and manages situational or other crises.
