What you'll do

• Cultivate relationships with prospective patients, families, physicians, nurses, discharge planners, case managers, and payer representatives
• manage an assigned territory
• complete patient assessments
• support the referral-to-admission process
• educate referral sources and community members about programs and services
• use market analysis to identify opportunities
• address admission barriers and follow up on admission variables
• and conduct in-services and presentations.
