What you'll do
- Complete initial assessments of patients, families, and home environments
- determine home health needs
- monitor and regularly reevaluate nursing needs
- and initiate or revise plans of care as patient status and needs change. Qualifications include current Illinois RN licensure and CPR
- home health experience and HCHB electronic medical record experience are preferred, and a valid Illinois driver’s license and auto insurance are required.
