What you'll do

• Follow the MDS calendar to schedule resident care conferences
• complete comprehensive care plans in PCC in a timely manner
• use medical records, resident and staff interviews, quarterly assessments, and consultant reports
• coordinate care plan meetings with the interdisciplinary team, residents, and resident representatives
• update existing care plans as needed.
