What you'll do

• Observe residents and document changes in behavior and mental or medical conditions
• communicate needs and concerns to lead staff and case managers
• support meals and daily living skills
• transport residents as requested
• monitor safety and assist with behavioral emergencies
• maintain records and charts
• support appointments, activities, and skills training
• provide prescribed therapeutic interventions
• assist with admissions, discharges, and medication administration when permitted by law and required by the unit.
