What you'll do

• Build therapeutic relationships with children and caregivers
• assess healthcare and social determinant needs
• coordinate interdisciplinary meetings and develop care and safety plans
• make referrals and connect families with health and community supports
• monitor care-plan goals
• coordinate transitions after hospital or emergency care
• document services and contacts
• complete discharge planning
• participate in quality assurance meetings
• and complete required training.
