Job overview
The Home Health Field Registered Nurse (RN) role is an onsite field position based in Athens, TX; compensation is not specified. The nurse provides patient-centered care in patients’ homes and works with interdisciplinary teams, patients, and families to support recovery and chronic disease management. What you'll do
- Conduct initial and ongoing comprehensive assessments
- determine eligibility and appropriate levels of home health care
- develop, implement, and document plans of care
- educate patients and families on self-care, nutrition, and dietary needs
- determine service scope and frequency
- provide preventative and rehabilitative nursing procedures
- follow infection-control precautions
- direct Home Health Aides and LPNs/LVNs
- prepare clinical and progress notes
- report changes to physicians and other personnel
- monitor cases for third-party payer requirements
- communicate with patients, families, and care professionals.
