The Certified Nursing Assistant (CNA), Hospice role is field-based in Kaukauna, WI, with a Monday–Friday schedule from 8:30 a.m. to 5:00 p.m. Pay is $18–$23 per hour. Moments Hospice provides comfort-focused care to patients in private residences, assisted living, and care facilities. The CNA provides hands-on personal care and companionship under the direction of an RN Case Manager. The role supports patient comfort and dignity and offers practical support to families as part of an interdisciplinary care team.

What you'll do

• Provide personal care including bathing, dressing, grooming, toileting, and skin care
• assist with lifting, transfers, positioning, range of motion, exercise, ambulation, dining, and nutrition
• follow each patient’s plan of care under the RN Case Manager
• provide comfort and companionship and family support
• maintain electronic medical records.
