What you'll do

• Develop and implement Individual Support Plans and Person-Centered Plans
• coordinate medical, dental, mental health, housing, and community integration services
• ensure regulatory and policy compliance
• maintain assessments, plans, and communication records
• collaborate with families, agencies, and involved parties
• participate in meetings, training, and program goal planning
• represent the organization in the community and stay informed about field developments.
