This role exists to provide outpatient rehabilitation services in an in-home setting, including private residences or independent living facilities. The PTA implements individualized treatment plans established by a primary Physical Therapist to facilitate increased independence and functioning for the older adult population.

What you'll do

• Provide in-home physical therapy treatments, implement individualized treatment plans, maintain timely documentation, and instruct patients and caregivers.
• Collaborate with the rehabilitation team and maintain equipment.
