Independently provides care for patients with acute and chronic illnesses. Takes full accountability for patient care and outcomes, seeking specialist consultation when needed. Engages with hospitalists when patients are hospitalized. Responsible for assessment, diagnosis, treatment, management, education, health promotion, and care coordination.
