The Health Homes Care Manager - Wayne role is onsite in Lyons, NY, with compensation of $21.75–$23.75 per hour. The position covers the Finger Lake region and requires day travel. Children’s Home of Wyoming Conference provides youth-focused support to help eligible children access healthcare, education, housing, and other services. The role coordinates comprehensive, strength-based care with children, families, and service providers. By assessing needs, developing care plans, responding to crises, and monitoring services, the Care Manager supports well-being and independence in the least restrictive environment.

What you'll do

• Coordinate case planning, care, and crisis response
• maintain relationships with DSS, schools, community agencies, and service providers
• assess family strengths and needs
• develop therapeutic relationships with families
• complete accurate reports and documentation
• monitor and evaluate services
• support intervention planning, skill development, and independent living.
