What you'll do

• Assess patients and collaborate with the care team to develop and implement plans of care
• make referrals based on patient needs
• teach self-care and provide appropriate rehabilitative measures
• supervise LPNs and HHAs
• complete timely and accurate documentation
• communicate patient progress to the clinical manager and care team
• plan and provide staff education
• perform assigned on-call responsibilities and patient/family services
• participate in clinical development and continuing education.
