MDS Registered Nurse Coordinator

Job overview

The MDS Registered Nurse Coordinator role is onsite in Ashley, MI; compensation is $47 per hour. Pioneer Health Care Management supports long-term care residents through assessment, care planning, and nursing services. The coordinator completes and submits federally mandated Resident Assessment Instruments and integrates assessment information into resident care plans. The role coordinates input across disciplines, monitors regulatory compliance, and supports nursing, admission, and reimbursement processes.

What you'll do

• Complete Resident Assessment Instruments accurately and within regulated time frames
• assess residents through examination, interview, and chart review
• coordinate significant-change assessments
• discuss care needs with interdisciplinary staff
• monitor RAI compliance
• enter information into electronic records
• schedule care-plan dates
• perform regular LPN/RN duties as needed
• support admissions, case management, Medicare meetings, and billing accuracy.
