What you'll do

• Conduct and document in-home client assessments, including the initial RN assessment
• advise on and update service plans
• instruct and supervise caregivers on plan duties and limits
• make supervisory visits and provide guidance
• evaluate caregiver competency and recommend training or support
• recognize changes in client condition and escalate concerns
• provide clinical guidance on a scheduled on-call rotation.
