Community-Based Rehabilitation Specialist / Case Manager

The Community-Based Rehabilitation Specialist / Case Manager role is onsite in Boise, ID, with stated pay of $23–$27 per hour. What you'll do

• Provide individualized community-based rehabilitation and case management in community and home settings
• use evidence-based, recovery-oriented interventions
• support daily living, social functioning, and community integration
• connect participants with psychiatric, medical, housing, financial, food, and other resources
• respond to crises using de-escalation and intervention techniques
• maintain documentation
• collaborate with a multidisciplinary treatment team.
