What you'll do

• Assess, advocate, link, and coordinate needed community-based services
• help individuals and families access medical, social, educational, and other support services
• provide counseling and therapeutic services as specified
• support client educational, legal, vocational, extracurricular, and health-related goals
• meet regularly with clients and families at home or residence and, as needed, at community locations.
