• Assist with social service assessments and care planning
• meet with residents and families
• help interpret residents’ social, psychosocial, and emotional needs
• provide information about Medicare and Medicaid
• coordinate discharge planning and referrals
• document concerns or grievances
• participate in care conferences
• coordinate with outside agencies
• and maintain confidentiality while following applicable regulations.
