The Clinical Pharmacist — Home Infusion role is an onsite, full-time position in Woburn, MA, paying $57–$64 per hour. New England Life Care is a nonprofit home infusion provider serving hospital systems across New England. The pharmacist provides clinical pharmacy services for home infusion patients, supporting safe medication use and transitions from hospital to home through prescription review, individualized care planning, monitoring, and communication with patients, caregivers, prescribers, and the healthcare team.

What you'll do

• Review prescriptions for accuracy and determine formulas and ingredients for dispensing
• assess patient suitability for home infusion
• help referral sources plan hospital-to-home transitions
• develop, implement, and monitor individualized plans of care and therapeutic goals
• advise healthcare team members on medication use, interactions, side effects, dosage, and storage
• educate patients and caregivers
• prepare, compound, and dispense medications under company procedures
• monitor orders for interactions, duplicate therapy, and appropriateness
• evaluate adverse drug reactions and recommend changes to prescribers.
