What you'll do

• Conduct client assessments and develop individualized treatment plans

• educate clients and families about substance use, recovery, and community resources
• provide crisis intervention and referrals as needed
• maintain confidential case documentation
• collaborate with healthcare and support professionals
• support relapse prevention and aftercare planning.
