What you'll do

• Provide general medical care and care coordination for adult, women’s health, pediatric, and geriatric populations
• conduct comprehensive evaluations and preventive assessments
• address acute and chronic primary care concerns
• establish diagnoses and care plans
• order or perform appropriate diagnostic tests
• coordinate post-discharge care
• conduct in-person visits in homes, skilled nursing facilities, and public locations
• participate in pop-up clinics
• document in the EMR
• provide alternating on-call coverage for urgent lab results and pharmacy inquiries
• travel locally, including driving up to 120 miles per day regularly.
