Designated Coordinator

The Designated Coordinator role is onsite in Saint Paul, MN, with compensation of $26.44–$29.81 per hour. Safeway Home Healthcare supports children and adults with disabilities across the Twin Cities through Minnesota 245D-licensed services. The coordinator leads daily service delivery and supports clients, families, and Direct Support Professionals (DSPs). The role helps ensure authorized services are delivered effectively, staffing and schedules are maintained, and service quality and client concerns receive appropriate attention.

What you'll do

• Serve as the primary contact for DSP questions, call-ins, and concerns
• coach and mentor staff and address performance and accountability
• complete incident reports and staff check-ins
• support clients and families
• assess new clients’ needs, home environments, and staffing compatibility
• ensure clients understand services and DSPs deliver authorized programs
• coordinate service starts, meet-and-greets, intake, interviews, hiring recommendations, and onboarding
• partner on staffing gaps, coverage, schedule approvals, and service utilization
• conduct quality check-ins and required visits
• escalate program or compliance issues
• participate in staffing meetings and provide weekly updates.
