The Hospice Registered Nurse Case Manager role is an onsite field position serving North San Diego County, CA, with patient visits requiring travel. The schedule is Monday–Friday, 8 a.m.–5 p.m., with two weekend shifts and two on-call shifts required monthly. Pay is $45–$61 per hour. The Elizabeth Hospice’s mission is to enhance quality of life for people nearing the end of life and those who grieve. The nurse assesses patients, develops and evaluates plans of care with the interdisciplinary team, patient, and family, and coordinates care with internal and external partners. The role also directs and supervises LVN and home health aide services, provides health instruction, and maintains timely electronic documentation.
