The Registered Nurse (RN) Clinical Manager, Transitional Care Unit role is onsite in Saint Paul, MN. Pay is $43–$48 per hour, depending on experience. The role supports Woodbury Health Care Center’s Transitional Care Unit and long-term care residents. The manager directs, coordinates, and evaluates nursing care and services while sharing responsibility for daily clinical operations. Through care-quality monitoring, accurate documentation, staffing coordination, and collaboration with facility leaders and care partners, the role supports resident care and the community’s work with seniors.

What you'll do

• Schedule or perform rounds to monitor and evaluate nursing care quality and appropriateness
• maintain accountability for charting and documentation of care, medications, and treatments
• assist with developing and implementing staffing plans and nursing schedules
• collaborate with the Executive Director and Director of Health Services to build relationships with prospects, customers, families, and care providers.
