What you'll do
- Plan, organize, and direct hospice care services for each patient and family, including instruction and evaluation in the home

- initiate and revise plans of care as needs change
- develop goals and palliative nursing actions with patients and families
- act as case manager when assigned by the Clinical Supervisor
- maintain and submit clinical notes
- document pain and symptom management changes and outcomes
- communicate patient needs and condition changes to physicians and obtain orders as required
- coordinate care with community health-related professionals
- supervise hospice aides.
