What you'll do

• Conduct initial and follow-up assessments
• develop individualized case plans with individuals served
• facilitate individual, group, and family therapy under supervision as applicable
• document progress and interactions
• monitor treatment progress
• communicate with staff and other professionals
• attend weekly treatment team meetings
• create aftercare plans
• liaise with insurers as requested
• assist with audits and documentation
• transport individuals occasionally
• represent TPC Wellness in court when necessary.
