What you'll do

• Complete initial and ongoing patient and family assessments
• develop and revise plans of care
• provide nursing care and prescribed medications and treatments
• counsel and educate patients and families
• document clinical notes and communicate changes to physicians
• coordinate with community health professionals and the hospice interdisciplinary group
• participate in on-call duties
• supervise ancillary personnel as required.
