Community Health Worker

What you'll do

• Enroll and manage eligible patient caseloads
• help patients navigate patient portals and the healthcare system
• provide education and support during agreed in-person visits
• support goal setting
• connect patients to social resources
• conduct at least two home visits and appointment accompaniment as needed
• document assessments and education
• collaborate with hospital and community teams
• and facilitate workshops and outreach events.
