Registered Nurse (RN) Care Coordinator

What you'll do

• Develop, implement, coordinate, and evaluate individualized healthcare plans
• collaborate with providers and practice staff to identify patients for care management
• conduct initial and periodic assessments
• educate patients and families and establish self-management goals
• monitor progress and revise plans as needed
• coordinate multidisciplinary care conferences and community referrals
• conduct post-discharge follow-up calls for high-risk patients
• coordinate transitions across care settings
• document in the electronic medical record and care management databases
• manage a caseload of approximately 50–100 patients.
