Case Manager, Askia

What you'll do

• Build professional relationships with clients
• assess needs and develop individualized care plans
• coordinate referrals and community resources
• advocate and assist with applications
• coordinate transportation and accompany clients to appointments as appropriate
• facilitate recovery-focused groups
• collaborate with the treatment team
• plan for discharge and aftercare
• conduct post-discharge follow-up
• assist with crisis intervention as needed
• complete required documentation.
