What you'll do

• Provide targeted case management and supports coordination
• collaborate on person-centered service planning
• link and coordinate clients with community services and agencies
• monitor service effectiveness and consumer satisfaction
• advocate for consumers
• coordinate with Medicaid Health Plans and other providers. LBSW-licensed staff may provide assessment and diagnosing services with master’s-level supervisor sign-off.
