The Nurse Specialist — Triage role is onsite in Lexington, KY. Compensation is $31.21 per hour. The position supports HealthFirst Bluegrass, a Federally Qualified Health Center whose mission is to provide compassionate, patient-centered care that strengthens community health. The role exists to serve as an initial clinical contact for patients and families, helping them navigate health concerns and access appropriate care. Through triage, care coordination, medication management, education, and advocacy, the nurse supports patients and connects them with providers and community resources.

What you'll do

• Serve as the first point of contact for patients and families with health questions or concerns
• support care coordination, triage, and medication management
• provide health education and counseling
• collaborate with providers and community health workers
• advocate for patients facing barriers such as transportation, prescriptions, or navigating the healthcare system.
