What you'll do

• Assess patients’ psychosocial status and communicate findings to the interdisciplinary group

• plan interventions based on evaluation findings
• counsel patients and families or caregivers as needed
• provide crisis intervention when necessary
• assess and educate the interdisciplinary group about cultural needs
